OLD
VERSION
Dear Parent/Guardian:
The charges for noon meals for the 2004-2005 school
year will remain the same as last year.
The rate will once again be $1.85 per meal for elementary age students
(grades K-6 – ages 5–11), and $2.00 for students older than 11 years. A thirty
day meal ticket will cost the following:
·
5-11 years old: $55.50
·
12–21 years old: $60.00
You will be notified one week before your son or
daughter’s ticket is to expire, so that a check written out to WSSB Meal
Program can be sent to the school to cover the cost of the next 30 day
ticket. Student meal tickets will be
kept in a locked area in the dining room and tickets will be punched each day
your son or daughter takes a meal. Payment
for meal tickets will need to be made prior to the start of the school
year. If you qualify for free and
reduced lunch through USDA, the meal will either be provided through this
program at a reduced rate or at no cost.
If you qualify for reduced priced meals, the business office will send
you the appropriate bill.
Enclosed is a form to request free and reduced meals through USDA. If you
believe your child qualifies, please complete the form. If you do not qualify, please sign the form
and write across the form do not
qualify. This will assist
us should we have to do any follow-up. In addition, the number of free and
reduced priced applications is the basis for which the school can receive other
grants, which benefit our students.
All meals served must meet patterns established by
the US Department of Agriculture. However if a child has been determined by
a doctor to be disabled and the disability would prevent the child from
eating the regular meal, the school will make any substitutions prescribed
by the doctor. Prescribed
substitutions must come in a written form signed by the doctor. These prescribed instructions will be located
in the health center and communicated to the food service staff.
Your cooperation is
appreciated. Thank you.
Sincerely,
Dean Stenehjem,
